2011 62TE Transmission Issue

I have owned this 2011 T&C since it had 10K miles on it. Now it has 130K. I've never been thrilled with the transmission overall, but since I can manually shift gears in certain instances I have learned to live with it. A couple years back, it would randomly throw a P0740 while cruising down the freeway, no indication anything was wrong. It would either go away or I would clear it and it would stay off for a long while. Recently, on a trip I was holding 4th gear while climbing and the transmission basically lost the gear, engine would rev but not go as if I was in neutral. Pulled to the side and turned vehicle off and on and it went into gear and shifted fine. It happened a couple of times, but only after manually shifting to 4th gear. If I only shifted between 5th and 6th it was fine, it just doesn't like to hold 4th very long when I manually shift to that gear.

Now, I'm getting the P0740 code more often. I changed the fluid and filter, it runs fine, shifts fine (auto mode) and doesn't seem to have an issue. I know some replace the speed sensors on the trans before pulling it for the TCC, I'm just wondering if there is also a TCM flash update as well? I've rented newer versions of the same van on business trips and have to say they shifted better than my van so I assume the programming has been fine tuned a bit since 2011.

Has anyone else had an issue where the trans seemingly went to neutral while manually shifting?
Has anyone had good luck with the speed sensor replacement (input/output)?

My transmission has been wonky for years. I don't manually shift it ever, but it throws codes occasionally. Last year it went into limp mode and I had to get a scan tool to reset it. There was an issue with the VLP solenoid. After I replaced the solenoid pack it seemed OK. On two occasions it has refused to shift out of first gear until the van was shut off and restarted. Both times were on very cold days in the winter. Once in a while it seems to shift extra harsh, like it couldn't quite make up it's mind. It still works, but swapping it out is on my list of things to do in the coming weeks/months. It makes noise when coming to a stop, the differential growls and there is noticeable whine from the planetaries. It's definitely tired...but it still works. As far as replacing the speed sensors, you can try that, but you can also try just cleaning the metal shavings off them too. Sometimes they accumulate a lot of metal powder over time, since they're basically all magnets.
